There may come a time when a Zad Treffer employee, former employee, or beneficiary is no longer able to manage their affairs or wants to allow another individual access to their Zad Treffer records. Should this be the case, Zad Treffer would require proper documentation authorizing this request.
Zad Treffer will accept valid Power of Attorney, Guardianship or Conservatorship documents. Valid documentation must include authority given including authority to update personal information. Once documentation is received, it will be reviewed for approval before any access is granted.
Zad Treffer will only accept the following types of documentation:
